SUMMARY:
The Phlebotomist is responsible for maintaining laboratory equipment, supplies, records, and procedures in accordance with OSHA, CLIA, CQI and other applicable standards.
The Phlebotomist is a member of the Patient Centered Medical Home health care team approach to delivering care in a planned, efficient and organized manner to include reviewing patient goals for the visit, making referrals to ancillary staff, and providing information about resources and programs available in their community.
The Phlebotomist is expected to work with the Medical and Laboratory Directors to improve the quality and efficiency of nursing and lab services; the Medical Assistant will receive supervision from the physicians, nurse practitioners, or physician assistants.
ESSENTIAL DUTIES AND RESPONSIBILITIES: 
  • They include, but are not limited to, the following:
  • Collects blood specimens from patients of all age groups and physical conditions by venipuncture or capillary technique as outlined by the Phlebotomy Manual.
  • Labels each specimen completely and accurately, as outlined in the Phlebotomy Manual, and legibly, such that the Accessioning Department can read everything on the label.
  • Completes test request forms completely and accurately. Also verifies client/patient test orders and client/patient billing information by telephone when necessary.
  • Centrifuges blood specimens as required by protocol and prepares specimens to maintain specimen integrity for transport to the appropriate laboratory or staging area.
  • Completes all required documentation on the same day as event requiring the documentation.
  • Performs clerical duties (e.g. maintaining records, generating and sending communications and reports, maintaining databases, entering data, faxing, answering phones, filing, etc.) completely, accurately and within established time frames.
  • Maintains appropriate inventory levels to ensure that specimens can be collected as required, all clerical duties can be performed, and the collection site can be kept clean.
  • Collects unobserved urine specimens, ensuring the necessary chain of custody protocol and following all procedures as outlined.
  • Responsible for the initial work-up of the patients either WI or appointments. This includes taking vital signs, obtaining heights and weights and obtaining urine specimens, hemoglobins, blood sugars and lead levels by capillary puncture, as needed;
  • Responsible for the correct recording of this date in the patient charts;
  • Assists in preparing the patient for examination by medical staff, assists during the examination as needed;
  • Assists in cleaning and sterilizing instruments used in the clinic, assuring labeling as to expiration dates and checking current status;
  • Assists in the general cleanup and maintenance of the patient examination rooms, work-up area and equipment;
  • Receive telephone calls and schedule appointments and follow-up;
  • Performs fingerstick hgb & lead, urine analysis and UCG’s, phlebotomy;
  • Assists with input of patient information into lab computer;
  • Assists with packaging of lab specimens for outside reference labs;
  • Follow proper procedure for providers to receive test results both normal and abnormal;
  • Assist with procedures when necessary;
  • Performs screening test, record the results and recognize gross abnormalities;
  • Conveys lab results to appropriate provider;
  • Prepare immunization, clinical indications and other reports;
  • Participates in continuing education on and off-site as appropriate and feasible;
  • Assists in obtaining and maintaining NCQA PCMH recognition;
  • Adheres to the delegated criteria of Laboratory, policy and procedures;
  • Participates in performance and training of OSHA Infection Control and Safety and adheres to the policies and procedures;
  • Performs other necessary duties as required by the health center to meet the goal of providing primary healthcare services.
QUALIFICATION REQUIREMENTS:

EDUCATION and/or EXPERIENCE:
This position requires an excellent knowledge of specimen handling needs/procedures, and must be proficient in drawing all age groups, and have at least one year phlebotomy experience. Must met all local and state requirements for phlebotomist as required. 
LANGUAGE SKILLS:
Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals. Ability to write routine reports and correspondence. Ability to communicate with patients and family members of various educational, socio-economic and cultural backgrounds. 
MATHEMATICAL SKILLS:
Ability to apply concepts of basic mathematics. 
REASONING ABILITY:
Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists. Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.
OTHER SKILLS AND ABILITIES:
Ability to operate basic office equipment, word processing and general computer skills.

The employee must occasionally lift and/or move patients of varying weight. Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception, and the ability to adjust focus.
Mental demands include the ability to learn, adapt to changes, and become proficient quickly, pay close attention to details, courteous and professional behavior, ability to work effectively in stressful working conditions.
